The Other (custom fields) tab allows the creation of custom fields that are not found in any other tabs, such as the custom fields shown in the image above, such as the first being a picklist field and the second being a text entry field..  These fields can also be named for use with tags so that any information entered here will appear in the generated report.

 

The instructions shown at the bottom of this tab describes how to switch to design mode to add or edit these fields.

 

These custom fields are specific to a report.  If you want to have them for all subsequent reports, simply create them in your template.  New reports created from that template will then have these custom fields available.
